Niall Ennis' 112th minute winner secured Mark Robins his first win as Potters boss and booked a spot in the Emirates FA Cup's last 32 as City defeated Sunderland at the Stadium of Light.
Tom Cannon's third minute penalty gave City an early lead, though an hour later Milan Aleksic levelled to send the tie to extra time.
Both sides had chances to score the game's third goal, though it was substitute Ennis who scored the decisive goal in extra time to extend the Potters' unbeaten run to four and secure a spot in Sunday's fourth round draw.
